Advanced ISR — Inkless Scar Revision — is an advanced clinical technique that stimulates the skin’s own collagen production and remodeling response in the scarred area. No pigment is deposited. The skin does the work; the procedure provides the controlled trauma that triggers the regenerative response.

The Procedure

The body’s own remodeling response.

Advanced ISR uses fine needles to create controlled microchannels in the scar tissue. These microchannels trigger the skin’s natural wound-healing cascade — collagen and elastin synthesis, capillary remodeling, and gradual reorganization of the scar’s structure. Over a series of sessions, scars that were once raised, depressed, or textured tend to soften toward the surrounding skin’s surface and pliability.

Because no pigment is introduced, ISR is appropriate for patients who don’t want a tattoo-based intervention, for scars that haven’t yet matured enough for camouflage tattooing, or for scars where the texture (not the color) is the main concern. It can also be used as a preparation step before later camouflage work — improving the tissue quality so pigment uptake is more predictable.

Consultation

Evaluation of the scar’s character — texture, color, age, depth — and assessment of whether ISR is appropriate. Some scars respond better to camouflage tattooing instead; some patients benefit from a combined approach. Dr. Rusnak will recommend the right path.

Treatment Sessions

Most patients need three to six sessions, spaced four to six weeks apart. Each session is one to two hours, with topical numbing applied throughout. The skin is mildly red for 24–48 hours afterward and may flake lightly during the healing window.

Evaluation & Maintenance

Progress is photographed at each session for comparison. Some scars continue improving for several months after the final session as the remodeling process completes. Maintenance sessions are optional and not always needed.

Who This Procedure Serves

For patients not yet ready for camouflage.

ISR is particularly useful for newer scars (six to twelve months old) that haven’t yet matured enough for pigmented camouflage work, for scars where the texture is the dominant issue rather than the color, and for patients who prefer a non-tattoo approach to scar improvement.

Common applications include acne scars, surgical scars where texture is the main concern, raised hypertrophic scars (with appropriate caution), and stretch marks in their earlier phase. ISR is not appropriate for active keloids, very recent scars still in inflammatory phase, or patients with active skin infections in the area.

How is this different from microneedling at a spa?

Standard cosmetic microneedling uses shorter needles at lower density and targets the upper skin layers for general rejuvenation. Advanced ISR is a paramedical protocol — Dr. Rusnak adjusts needle depth and density specifically to the scar’s depth and tissue character, and treats it as a clinical procedure with measurable progression rather than a cosmetic facial.

How many sessions will I need?

Most patients need three to six sessions. Newer scars and texture-driven scars often respond faster; deeper or older scars may need more sessions. Dr. Rusnak will give you a treatment plan at consultation.

Does it hurt?

Topical numbing is applied throughout. Most patients report mild discomfort — a pressure or scratching sensation rather than sharp pain. The numbing is effective.

What’s the downtime?

Minimal. The treated area is mildly red and slightly swollen for 24–48 hours. Light flaking can happen during the first week of healing. You can typically return to work the next day.

Can ISR be combined with other treatments?

Yes. Dr. Rusnak often combines ISR with later scar camouflage tattooing — using ISR to improve the tissue quality first, then pigmenting once the scar has remodeled. This combined approach can give better results than either treatment alone.
